Please use the following links in order to complete your student/s registration forms.  You will be able to complete each form for multiple children. Once you have filled out and submitted the forms, we will have access to them on our database.  
Please disregard any forms that are designated for a grade that you do not have a student enrolled in (ex. Grade 6 Health Curriculum Form).  If you have filled out a health form online in the last two days for school teams that were travelling for games, you do not need to fill out the SHSAA Health Form again.
You will now notice that when you fill out the Student Fees/Yearbook Form, you are given the option to pay these fees online using your credit card.  There are service charges applied to the fees if you choose this option, which are reflected on the form.  You are not required to pay by this method, and may pay by cash or cheque as in other years.  If you are paying cash or cheque, the fees are as follows:
Jr. Student Fee ($10.00)
Sr. Student Fee ($20.00)
Family Fee (3 students or more) ($50.00)
Yearbook ($40.00 each)
If you wish to pay by cheque, please make it out to OHS Admin and submit it to the office.

We will not be able to send the Student Registration Form with all the current  information we have for each student on file (home address, phone number, parent names, etc.) until later this month.  The Division office is currently updating this form so we do not have access to it.  We will have it sent home to to parents/guardians as soon as possible.   This will be coming home in paper form with your student/s and will need to be returned to the office.
